So, here's my final theory. If you've DVRed the episode and haven't seen it yet.. DON'T CONTINUE!
I've been convinced for well over a month now that the entire show is nothing more than a game being played by these two entities. All of the losties are simply pawns in their game, nothing more, and the game has played out time and time again by the players (as evidenced by the hundreds of names scratched out in the seaside cave).
Last night, we see Jacob and MIB as teenagers were playing SENET - an ancient egyptian game similar to Mancala type games, and probably a precursor to backgammon.
MIB bitches Jacob makes an illegal move, Jacob bitches MIB made up the rules (which is interesting as there are no official rules, just theories of how the game was played). To which MIB replies...
"One day Jacob you'll have to make up your own game and then everyone will have to play by your rules."
Bam... to me, that one line summarizes the entire show.
Dharma folks, the Black Rock crew, the US military, all have been pawns in their games, each game similar to the last but the rules changed a little.
Senet, Mancala, backgammon.. all of these games have the same objectives.. get all your pawns home, and keep the opponent from doing the same. In backgammon, you can put pieces in limbo until your opponent can roll them out.
Not unlike when the 6 were "rescued". They weren't rescued, they were blots in backgammon tossed out of play! Not unlike the "sideways" flashes this season. They're out of play, and one player is trying to "roll them back in" through Desmond. The island is one side of the game board. The "alternate reality" is the other side of the game board.
This show is absolutely f***ing brilliant. From the very first episodes they blatantly dangle answers right in front of your face, but sidetrack you with things that aren't as important but seem to be! This final season has turned out to be the most enjoyable for me. From the trees getting tossed through the air in the pilot episode, to Locke banging on the hatch door crying, only to have the light come on, to it looking like Ben is the owner of the island, to realizing he's just another pawn in a sick twisted game... I'm really going to miss this show.
But I can't wait to see who wins the game!

The liquid that Jacob drank, made him immortal? Well at least sort of. Now we know how the "smoke moster/MIB/fake Lock" came about.
What confused me about this episode, I thought the "woman" who raised the twins was also immortal since she was the guardian of the light in the tunnel. If she was the "original" guardian, then that means that originally, the guardian was both evil & good up until the "twins" were born?

I totally agree. Good prediction and I can't wait to see how it all plays out.
Lots of unanswered questions still, but I absolutely LOVE this show. I have Season 1 to present on my hard drive, ready to watch after the last episode airs. Can't wait to watch it again and see what I have missed.
But which one's evil and which one's good? And is "evil" and "good" the right way to differentiate them. It doesn't seem like Smokie is really evil. And Jacob looks kinda like a wussie now.
"Mom" was immortal. She didn't age over the 40+ years of this episode. Smokie snuck up behind "Mom" and stabbed her before she could speak. They've established that that's one way to kill them.
It does seem like they're leading up to your prediction but it's almost expected so maybe they'll do something different.

Smokie (love that name) has been doing all the killings so far. Who has Jacob killed? He appears to be a "collector" as in finding or selecting people who will come to the island to satisfy his & smokie demi-god needs.
when the Man in Black wanted to get off the island badly he was not yet Smokey. Smokey was the Mom the whole time the boys were growing up (she wiped out the village, too) and she never wanted to get off of the island. so i don't buy that Smokey wants off of the island. John Locke is now Smokey and the soul and character of Locke is dead; this is just Smokey appearing to look and sound like Locke.
i suspect that Desmond will end up killing John Locke/Smokey (or perhaps Claire will do that) and that Desmond will be the person to go down the cave of light and that the show will end as he descends before we find out what happens.
