I've updated to itunes 8.0.2, and iPhone OS 2.2 - iTunes is showing a red '2' against Applications in the sidebar (though ny iphone says they're all up to date) and no clue as to what you do to update applications from iTunes itself.

Hah - even though it says nothing about it in the iTunes Help, when the Applications panel is showing, there's a little Check For Updates link in the bottom right which I never noticed. Of course, it still doesn't exatly work properly, but it does the job after a couple of tries.
